Responsibilities
- Provide comprehensive virtual patient assessments and clinical evaluations via telehealth platform
- Develop and implement personalized care plans for chronic disease management and acute conditions
- Coordinate multidisciplinary care teams and facilitate seamless transitions between healthcare settings
- Document patient encounters accurately in EHR systems while maintaining HIPAA compliance
- Educate patients on health maintenance, medication adherence, and preventive care strategies
- Respond to urgent virtual consultations and triage complex clinical scenarios
- Participate in continuous quality improvement initiatives and clinical protocol development
Qualifications
- Active RN license in North Carolina (or compact state eligibility)
- Bachelor of Science in Nursing (BSN) required; MSN preferred
- Minimum 3 years clinical experience in acute care, home health, or telehealth
- Proficiency with electronic health records and telehealth platforms (e.g., Teladoc, Amwell)
- Strong critical thinking and decision-making abilities in virtual settings
- Certification in telehealth or case management highly desirable
- Exceptional communication skills with demonstrated patient-centered approach
