Responsibilities
- Provide comprehensive virtual patient assessments and care coordination via telehealth platforms
- Monitor and interpret patient data from remote monitoring systems
- Collaborate with interdisciplinary teams to develop and implement personalized care plans
- Deliver patient education and support for chronic disease management
- Document clinical encounters accurately in EMR systems
- Respond to emergency situations and escalate critical cases appropriately
- Maintain strict HIPAA compliance and data security protocols
Qualifications
- Active California RN license (required)
- BSN degree (ADN candidates with 3+ years experience considered)
- 2+ years of clinical nursing experience in acute care or telehealth
- Proficiency with telehealth technology and EMR systems
- BLS and ACLS certifications
- Strong critical thinking and decision-making abilities
- Exceptional communication skills for virtual patient interactions
- Ability to work independently with minimal supervision
