Responsibilities
- Provide telehealth patient assessments and care coordination
- Monitor patient progress and adjust care plans remotely
- Collaborate with interdisciplinary healthcare teams
- Document patient interactions accurately in EMR systems
- Delicate patient education and health coaching via digital platforms
- Respond to urgent patient inquiries during assigned shifts
- Maintain HIPAA compliance in all virtual interactions
Qualifications
- Active Arizona RN license in good standing
- Minimum 2 years of clinical nursing experience
- BLS/CPR certification required
