Responsibilities
- Monitor patient vital signs and health status via telehealth platforms
- Coordinate care plans between patients, families, and medical teams
- Provide emotional support and patient education remotely
- Maintain accurate electronic health records and documentation

Qualifications
- Active Texas CNA certification in good standing
- Minimum 1 year clinical experience preferred

- Exceptional communication and empathy skills

- High school diploma or equivalent required
- Valid CPR certification
