Responsibilities
- Provide comprehensive remote patient assessments and care coordination via secure telehealth systems
- Monitor patient vitals, medication adherence, and recovery progress through digital health platforms
- Collaborate with physicians and specialists to develop and implement personalized care plans
- Document patient encounters accurately in electronic health records (EHR) systems
- Educate patients and families on health management and preventive care
- Respond to urgent patient inquiries and escalate critical situations appropriately
- Participate in virtual multidisciplinary rounds and quality improvement initiatives
Qualifications
- Active California RN license (unrestricted)
- Minimum 3 years of clinical nursing experience in acute care or telehealth
- BLS and ACLS certifications required
- Proficiency with EHR systems and telehealth technology
- Strong critical thinking and decision-making abilities
- Exceptional communication skills for virtual patient interactions
- Ability to work independently while collaborating effectively in remote teams
- BSN degree preferred (ADN with relevant experience considered)
