Responsibilities
- Conduct comprehensive virtual patient assessments via telehealth platform
- Develop and implement personalized care plans for chronic disease management
- Provide patient education and health coaching via secure video consultations
- Collaborate with multidisciplinary care teams to ensure continuity of care
- Document clinical encounters accurately in electronic health records (EHR)
- Respond to patient inquiries and triage urgent situations appropriately
- Participate in quality improvement initiatives and peer review processes
Qualifications
- Active North Carolina RN license (unrestricted)
- Minimum 2 years of clinical nursing experience
- Proficiency with telehealth platforms and EHR systems
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Experience with chronic disease management preferred
- BLS/CPR certification required
- Ability to work independently and manage time effectively
