Responsibilities
- Deliver comprehensive telehealth assessments and patient consultations via secure virtual platforms
- Develop and implement individualized care plans for chronic disease management
- Coordinate care transitions and collaborate with community healthcare providers
- Document patient encounters accurately in EHR systems while maintaining HIPAA compliance
- Provide health education and coaching to patients via telehealth modalities
- Participate in continuous quality improvement initiatives for remote care delivery
- Respond to patient inquiries and triage urgent situations through virtual channels
Qualifications
- Active Arizona RN license (unrestricted) required
- Minimum 2 years clinical nursing experience
- Telehealth or virtual care experience strongly preferred
- BLS/CPR certification (current)
- Proficiency with EHR systems and telehealth platforms
- Strong assessment and critical thinking skills
- Excellent communication abilities for virtual patient interactions
