Responsibilities
- Conduct virtual patient assessments and triage via telehealth platforms
- Develop and implement personalized care plans remotely
- Provide patient education and health coaching through digital channels
- Collaborate with interdisciplinary care teams via secure communication tools
- Monitor and document patient progress in EMR systems
- Respond to urgent patient inquiries within agreed SLAs

Qualifications
- Active RN license in the state of Arizona
- Minimum 2 years of clinical nursing experience
