Responsibilities
- Conduct virtual patient assessments and care coordination
- Monitor and document patient vitals and health status remotely
- Coordinate with healthcare teams for care plan adjustments
- Provide telephonic patient education and support
- Manage electronic health records (EHR) documentation
- Respond to patient inquiries and emergency protocols
- Collaborate with nurses and physicians for continuity of care
Qualifications
- Active Arizona CNA certification required
- Minimum 1 year clinical experience
- Strong telehealth technology proficiency
- Excellent communication and empathy skills
- Ability to work independently in remote setting
- Basic computer literacy and EHR experience
- Valid Arizona driver's license (for occasional local visits)
- Background check and drug screening clearance
