Responsibilities
- Assess patient symptoms and prioritize medical urgency using established triage protocols
- Provide expert telephone consultations to patients and healthcare providers
- Maintain accurate electronic health records (EHR) with detailed clinical documentation
- Collaborate with physicians and multidisciplinary teams for seamless patient transitions
- Educate patients on appropriate care pathways and self-management strategies
- Monitor patient outcomes and adjust care plans as needed
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Kentucky with minimum 2 years ER/urgent care experience
- BLS and ACLS certifications required
- Proven expertise in rapid patient assessment and decision-making
- Exceptional communication skills with diverse patient populations
- Proficiency in EHR systems (Epic preferred)
- Ability to work independently in high-pressure environments
- Bachelor's degree in Nursing preferred
