Responsibilities
- Conduct comprehensive patient assessments: Utilize evidence-based guidelines to evaluate symptoms and determine the urgency of care requirements.
- Prioritize clinical care: Triage incoming calls and inquiries to ensure patients receive the appropriate level of medical attention immediately.
- Facilitate referrals: Coordinate seamlessly with physicians, specialists, and healthcare facilities to arrange follow-up appointments and treatments.
- Document accurately: Maintain detailed and up-to-date electronic health records (EHR) for all patient interactions and triage notes.
- Provide health education: Educate patients on self-care management, medication adherence, and preventive health measures.
- Collaborate with the care team: Communicate effectively with providers to discuss complex cases and ensure continuity of care.
Qualifications
- Current RN License: Active and unencumbered Registered Nurse license in the state of Arizona.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certification required.
- Experience: Minimum of 2 years of clinical nursing experience, with specific experience in triage or telehealth preferred.
- Communication Skills: Exceptional verbal and written communication skills with the ability to convey complex medical information clearly.
- Decision Making: Strong critical thinking and clinical judgment skills to make timely, accurate decisions under pressure.
- Technology Proficiency: Comfortable navigating EMR systems and telehealth platforms.
