Responsibilities
- Provide direct patient care, including bathing, dressing, grooming, and ambulation assistance to ensure patient dignity and comfort.
- Monitor vital signs (blood pressure, temperature, pulse, respiration) and report any significant changes in patient condition to the charge nurse immediately.
- Administer prescribed medications and treatments in strict accordance with safety protocols and documentation standards.
- Assist with patient mobility, transfers, and positioning to prevent falls and pressure ulcers.
- Maintain accurate and timely documentation of care activities in electronic health records (EHR).
- Collaborate with the interdisciplinary team to develop and update individualized patient care plans.
- Foster a safe, clean, and therapeutic environment for residents and patients.
Qualifications
- Current and active Certified Nursing Assistant (CNA) license in the state of Tennessee.
- Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) preferred.
- Previous experience in a long-term care, hospital, or home health setting is preferred, but new graduates are encouraged to apply.
- Strong compassion, patience, and excellent interpersonal communication skills.
- Ability to work independently and collaboratively during night shifts.
- Physical stamina to stand for extended periods and lift patients as required.
