Responsibilities
- Provide compassionate patient care in home-based settings via telehealth coordination
- Monitor vital signs and document patient progress using EHR systems
- Assist with daily living activities and medication management
- Collaborate with RNs and care teams to develop personalized care plans
- Respond to emergency situations following established protocols
- Maintain accurate patient records and documentation
- Support families through virtual education and resource coordination
Qualifications
- Active Arizona CNA certification in good standing
- Minimum 1 year of clinical experience preferred
- Proficiency with telehealth platforms and EHR systems
- Strong communication skills for remote patient interactions
- Valid CPR certification
- Ability to work independently and manage time effectively

- Reliable transportation for occasional in-person visits
