Responsibilities
- Provide direct patient care including bathing, dressing, and mobility assistance
- Monitor vital signs and document patient observations accurately
- Assist with medication administration under RN supervision
- Coordinate telehealth visits and remote patient monitoring
- Maintain electronic health records (EHR) systems remotely
- Support patient education and discharge planning
- Collaborate with interdisciplinary care teams
Qualifications
- Active Texas CNA certification required
- Minimum 1 year clinical experience preferred
- Proficient with telehealth technology and EHR systems
- Valid CPR certification
- Strong communication and problem-solving skills
- Ability to work independently and adapt to flexible environments
