Responsibilities
- Provide Direct Patient Care: Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility.
- Monitor Vital Signs: Accurately record and report patient vital signs, including blood pressure, temperature, and pulse.
- Medication Assistance: Support patients with medication reminders and adhere to all medication administration protocols.
- Communication: Serve as a liaison between patients, families, and the nursing team to ensure comprehensive care.
- Housekeeping & Hygiene: Maintain a clean, safe, and sanitary environment for patients and staff.
- Emotional Support: Offer compassionate companionship and emotional support to patients and their families.
- Documentation: Complete accurate and timely documentation of patient care in electronic health records (EHR).
Qualifications
- Certification: Active and unencumbered Certified Nursing Assistant (CNA) license in the state of Minnesota.
- Education: High school diploma or GED required.
- CPR Certification: Current Basic Life Support (BLS) certification for healthcare providers.
- Experience: Minimum of 1 year of experience in a long-term care or home health setting preferred.
- Skills: Strong interpersonal and communication skills with a focus on patient-centered care.
- Physical Requirements: Ability to lift, stand, walk, and bend for extended periods; ability to lift up to 50 lbs.
