Responsibilities
- Provide high-quality patient care including bathing, dressing, and mobility assistance
- Monitor vital signs and document patient observations via our remote EHR system
- Assist with medication administration under RN supervision
- Coordinate care plans with interdisciplinary teams (remote collaboration available)
- Respond to patient call lights and ensure safety protocols
- Maintain accurate electronic health records from remote locations
- Support emotional well-being through compassionate communication
Qualifications
- Active Pennsylvania CNA certification required
- Minimum 1 year recent clinical experience in long-term care or home health
- Ability to work flexible shifts (days, evenings, weekends)
- Proficient with electronic health record systems
- Strong interpersonal and communication skills
- Current CPR/BLS certification
- Reliable transportation for in-person care rotations
- Ability to lift 50+ pounds with proper techniques
