Responsibilities
- Patient Hygiene: Assist patients with daily living activities, including bathing, grooming, dressing, and toileting with dignity and respect.
- Vital Signs: Monitor and record patient vital signs (blood pressure, temperature, pulse, respirations) accurately and in a timely manner.
- Medication Support: Assist with medication reminders and ensure patients are taking prescribed medications as directed by licensed nursing staff.
- Mobility Assistance: Safely transfer patients between beds, wheelchairs, and chairs using proper body mechanics and assistive devices.
- Documentation: Maintain accurate and up-to-date patient records in the Electronic Health Record (EHR) system.
- Communication: Act as a liaison between patients, families, and the healthcare team, reporting changes in condition immediately.
- Sanitation: Maintain a clean, safe, and sanitary environment for all patients.
Qualifications
- License: Valid and current California State CNA Certification.
- Training: Completion of a state-approved Nursing Assistant program.
- Certifications: Current Basic Life Support (BLS) certification (ACLS preferred).
- Experience: Minimum of 1 year of experience in home health or long-term care is highly preferred.
- Skills: Strong interpersonal and communication skills with the ability to work independently.
- Physical Requirements: Ability to lift, push, and pull patients up to 250 lbs; ability to stand for extended periods.
- Background: Clean criminal background check and drug screen required.
