Responsibilities
- Provide direct patient care, including assistance with bathing, dressing, and personal hygiene.
- Monitor and document patient vital signs, intake/output, and changes in condition.
- Assist with mobility and transfers using proper body mechanics to ensure safety.
- Maintain a clean, safe, and therapeutic environment for residents and patients.
- Communicate effectively with the nursing team, patients, and family members regarding care plans.
- Administer medications and treatments under the supervision of a Registered Nurse (RN).
Qualifications
- Active and current CNA license in the state of Ohio.
- Current Basic Life Support (BLS) certification.
- High school diploma or GED required.
- Previous experience in home health, hospice, or long-term care is highly preferred.
- Strong interpersonal and communication skills with a patient-centered approach.
- Reliable transportation and ability to pass a background check.
