Responsibilities
- Provide direct patient care, including bathing, dressing, and grooming assistance, to ensure patients maintain personal hygiene and dignity.
- Monitor and record vital signs, such as blood pressure, pulse, and temperature, reporting any significant changes to the RN or charge nurse immediately.
- Assist patients with mobility, transfers, and walking using proper body mechanics and assistive devices to prevent injury.
- Document patient care activities, including meal intake, output, and behavioral changes, in accordance with facility protocols.
- Communicate effectively with patients, families, and the interdisciplinary healthcare team to provide updates on patient conditions and needs.
- Maintain a clean, safe, and therapeutic environment for patients by adhering to infection control and safety standards.
Qualifications
- Current and active Pennsylvania CNA License required.
- CPR and Basic Life Support (BLS) certification from the American Heart Association (AHA) or Red Cross.
- High School Diploma or GED equivalent.
- Previous experience in a long-term care or hospital setting is preferred but not required.
- Strong interpersonal and communication skills with a compassionate and patient-centered approach.
- Ability to stand, walk, and lift up to 25 lbs frequently throughout the shift.
