Responsibilities
- Provide Direct Care: Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting, ensuring dignity and comfort.
- Monitor Health: Observe and report changes in patient condition, vital signs, and behavior to the nursing staff immediately.
- Medication Support: Assist with medication reminders and administration under the direct supervision of a Registered Nurse or Licensed Practical Nurse.
- Documentation: Maintain accurate and timely documentation of care plans, treatments, and patient progress notes.
- Communication: Collaborate effectively with the healthcare team, patients, and families to ensure holistic care.
- Environment Safety: Ensure the patient environment is clean, safe, and free from hazards to prevent falls and infections.
Qualifications
- License: Current, active Certified Nursing Assistant (CNA) license in the State of California.
- Certification: Basic Life Support (BLS) certification is required.
- Experience: Previous experience in home health, geriatric care, or long-term care is preferred.
- Physical Ability: Ability to lift, transfer, and position patients; physical stamina to stand and walk for extended periods.
- Skills: Strong interpersonal and verbal communication skills with a patient-centered approach.
- Attitude: Compassionate, reliable, and eager to work in a fast-paced healthcare setting.
