Responsibilities
- Provide direct patient care, including assistance with bathing, dressing, and grooming.
- Monitor vital signs and report any changes in patient condition to the nursing team immediately.
- Assist with mobility and transfers, ensuring patient safety at all times.
- Document patient care activities, intake, and output accurately and timely.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Maintain a clean, safe, and organized patient environment.
- Communicate effectively with patients and their families to provide emotional support and education.
Qualifications
- Current, active Texas CNA or LPN license in good standing.
- Certification in Basic Life Support (BLS) and CPR is required.
- Previous experience in a hospital, long-term care, or home health setting preferred.
- Strong communication and interpersonal skills with a focus on patient-centered care.
- Ability to stand for long periods and lift up to 50 lbs as needed.
- Reliable transportation and ability to pass a background check and drug screening.
