Responsibilities
- Provide direct patient care, including assistance with bathing, dressing, grooming, and toileting.
- Monitor and record vital signs, such as blood pressure, temperature, and pulse rates accurately.
- Assist patients with mobility, transfers, and positioning to ensure safety and prevent falls.
- Communicate changes in patient conditions or concerns to the supervising nursing staff immediately.
- Prepare and serve meals according to individual dietary restrictions and preferences.
- Maintain a clean, safe, and sanitary environment for patients and residents.
- Document patient care activities and progress in electronic health records (EHR) as required.
Qualifications
- Certified Nursing Assistant (CNA) or Licensed Practical Nurse (LPN) license required by the State of Minnesota.
- Current Basic Life Support (BLS) certification.
- Previous experience in a healthcare or long-term care setting preferred.
- Strong verbal and written communication skills.
- Ability to stand, walk, and lift for extended periods.
- Compassionate, patient-centered approach to care.
- Reliable transportation and willingness to pass a background check and drug screen.
