Responsibilities
- Provide direct patient care including bathing, dressing, and mobility assistance
- Monitor vital signs and report changes to nursing staff
- Assist with patient documentation using our digital health records system
- Support remote patient monitoring initiatives for post-discharge follow-up
- Collaborate with interdisciplinary teams to develop care plans
- Maintain a clean, safe patient environment

Qualifications
- Active North CNA certification in good standing
- Minimum 1 year acute care or hospital experience
- Basic Life Support (BLS) certification required
- Proficiency with electronic health records (EHR) systems
- Strong communication and interpersonal skills
- Ability to work independently with minimal supervision
- Flexibility to adjust between in-person and remote tasks
