Responsibilities
- Provide direct patient care and assistance with Activities of Daily Living (ADLs) including bathing, dressing, and grooming.
- Monitor vital signs (blood pressure, pulse, respiration) and report any significant changes to the nursing staff immediately.
- Assist with mobility, transfers, and positioning to ensure patient safety and comfort.
- Maintain a clean, organized, and sanitized patient environment in accordance with health and safety regulations.
- Document patient care activities accurately and timely in the Electronic Health Record (EHR) system.
- Collaborate effectively with the interdisciplinary care team to develop and implement individualized care plans.
- Communicate clearly with patients, families, and healthcare providers to ensure holistic care.
Qualifications
- Current, unencumbered California State CNA license required.
- BLS (Basic Life Support) certification from the American Heart Association required.
- Previous experience in a hospital, home health, or long-term care setting is preferred.
- Strong interpersonal and communication skills.
- Ability to lift, push, and pull patients weighing up to 250 lbs as required.
- Compassionate, patient-focused attitude with a strong work ethic.
- Reliable transportation and willingness to pass a background check.
