Responsibilities
- Assist clients with daily activities including meal preparation, medication reminders, and personal hygiene
- Provide companionship through conversation, games, and accompanying clients to appointments
- Monitor clients' physical and emotional well-being, reporting changes to care coordinators
- Perform light housekeeping tasks such as laundry, tidying, and meal cleanup
- Transport clients to medical appointments and social outings using personal vehicles
- Maintain accurate care documentation and follow individualized care plans
Qualifications
- High school diploma or equivalent (GED accepted)
- Valid Wisconsin driver's license and reliable transportation
- Ability to pass background check and health screening
- Basic computer literacy for documentation
- Strong communication skills and active listening abilities
- Physical ability to assist clients with mobility (bending, lifting up to 50 lbs)
- CPR/First Aid certification (provided if not currently held)
