Responsibilities
- Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting assistance.
- Prepare nutritious meals and assist with meal planning based on dietary restrictions.
- Provide medication reminders and support with daily medication management.
- Offer companionship and engage clients in conversation, hobbies, and social activities.
- Perform light housekeeping duties such as laundry, vacuuming, and changing linens.
- Accompany clients to medical appointments, shopping trips, or community outings.
- Monitor and report any changes in the client’s physical or mental condition to the case manager.
Qualifications
- Valid Driver’s License and reliable personal transportation required.
- CPR and First Aid certification preferred.
- Previous experience as a Home Health Aide, CNA, or Personal Care Assistant is highly preferred.
- Strong interpersonal and communication skills with a patient and empathetic demeanor.
- Ability to pass a background check and drug screening.
- Valid state certification or licensure (where applicable).
