Responsibilities
- Conduct comprehensive phone triage assessments to evaluate patient symptoms and medical history.
- Prioritize patient care based on acuity and urgency to ensure timely medical intervention.
- Document all patient interactions and triage decisions accurately in the electronic health record (EHR).
- Coordinate patient referrals to appropriate providers, specialists, or urgent care facilities.
- Collaborate with physicians and nursing staff to resolve complex patient inquiries and care issues.
- Educate patients on self-care management, medication adherence, and preventive health practices.
- Monitor patient follow-ups to ensure treatment plans are being followed effectively.
Qualifications
- Current, unencumbered Registered Nurse (RN) License in the State of Texas.
- Minimum of 2-3 years of clinical nursing experience, with specific experience in triage or urgent care preferred.
- Valid Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Strong critical thinking skills and the ability to make rapid, accurate clinical decisions under pressure.
- Excellent verbal and written communication skills for clear patient education.
- Proficiency with electronic health record systems and medical terminology.
- Ability to remain calm and professional in high-stress situations.
