Responsibilities
- Patient Assessment: Evaluate patient symptoms via phone, portal, or in-person to determine the urgency of care and appropriate medical response.
- Care Coordination: Coordinate referrals to specialists, emergency services, or primary care physicians based on clinical findings.
- Documentation: Maintain accurate and detailed electronic health records (EHR) documenting patient history, presenting complaints, and triage decisions.
- Protocol Adherence: Ensure all nursing and triage protocols, including HIPAA compliance, are strictly followed during patient interactions.
- Provider Support: Communicate critical findings and patient updates effectively to physicians and on-site nursing staff.
- Patient Education: Provide patients with clear instructions regarding medications, follow-up care, and home health resources.
Qualifications
- Licensure: Current and valid Registered Nurse (RN) license in the State of Texas.
- Education: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Experience: Minimum of 1-2 years of experience in triage, emergency room, or urgent care nursing required.
- Communication: Exceptional verbal and written communication skills with the ability to de-escalate anxious patients.
- Technology: Proficient in using electronic health record systems and triage software.
