Responsibilities
- Provide high-quality virtual patient assessments and care coordination via telehealth platform
- Monitor and document patient health data using advanced electronic health records
- Collaborate with interdisciplinary teams to develop patient care plans
- Educate patients and families on health management and preventive care
- Adhere to HIPAA compliance and hospital protocols in all virtual interactions
- Participate in continuous quality improvement initiatives
- Respond to emergency situations and escalate critical cases appropriately
Qualifications
- Valid North Carolina Registered Nurse (RN) license required
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
