Responsibilities
- Deliver patient-centered virtual nursing care through telehealth platforms for diverse patient populations
- Conduct comprehensive health assessments and develop personalized care plans remotely
- Collaborate with interdisciplinary teams to coordinate patient care transitions and interventions
- Educate patients on health management, medication protocols, and preventive care measures
- Accurately document patient encounters in electronic health records (EHR) systems
- Monitor patient progress and adjust care plans based on clinical outcomes and feedback
- Participate in continuous quality improvement initiatives and telehealth protocol development
Qualifications
- Active Arizona Registered Nurse (RN) license (or ability to obtain immediately)
- Associate's or Bachelor's degree in Nursing (ADN or BSN required)
- New graduates and nurses with less than 1 year of experience encouraged to apply
- Basic proficiency with telehealth technology and EHR systems
- Strong critical thinking and clinical decision-making abilities
- Excellent communication skills with demonstrated empathy and cultural competence
- Ability to maintain HIPAA compliance in remote work environments
- Flexibility to work rotating shifts including evenings, weekends, and holidays
