Responsibilities
- Assess patient calls using standardized triage protocols to determine urgency and appropriate care level
- Provide clinical guidance and education to patients via phone, telehealth, and secure messaging platforms
- Collaborate with physicians and care teams to coordinate patient transitions and follow-up care
- Maintain accurate electronic health records with detailed documentation of all interactions
- Participate in continuous quality improvement initiatives for triage processes
- Respond to medical emergencies with rapid intervention protocols and resource mobilization
Qualifications
- Active Hawaii RN license (or compact state license with ability to obtain HI license)
- Minimum 2 years clinical experience in emergency or urgent care setting
- BLS and ACLS certifications required; TNCC preferred
- Proficiency in Epic EMR and telehealth platforms
- Exceptional critical thinking and decision-making skills under pressure
- Strong written and verbal communication abilities
- Flexibility to work evenings, weekends, and holidays as needed
