Responsibilities
- Deliver virtual patient assessments and care coordination via telehealth platforms
- Assist with medication management reminders and health monitoring
- Document care plans and patient progress in electronic health records
- Collaborate with RNs and care teams to ensure continuity of care
- Provide emotional support and health education to patients and families
- Respond to patient inquiries and triage urgent concerns
- Participate in interdisciplinary care conferences remotely
Qualifications
- Active Virginia CNA certification in good standing
- 1+ years of clinical experience in home health or long-term care
- Proficiency with telehealth technology and EHR systems
- Strong communication and documentation skills
- Ability to work independently with minimal supervision
- CPR certification (or willingness to obtain)
- Valid driver's license and reliable transportation for occasional site visits
