Responsibilities
- Assess patient symptoms via phone and in-person to determine urgency and appropriate care level
- Collaborate with physicians to develop triage protocols and streamline patient flow
- Maintain accurate electronic health records (EHR) documentation
- Provide education to patients regarding home care and follow-up procedures
- Coordinate with emergency departments and specialty clinics for seamless transitions
- Monitor patient vital signs and clinical status during triage process
- Participate in continuous quality improvement initiatives
Qualifications
- Active NJ RN license with minimum 2 years clinical experience
- BLS/ACLS certification required; TNCC preferred
- Proven triage experience in emergency or urgent care setting
- Exceptional critical thinking and decision-making skills
- Strong proficiency in EHR systems (Epic preferred)
- Excellent communication and de-escalation abilities
- Bachelor of Science in Nursing (BSN) preferred
