Responsibilities
- Provide personal care assistance including bathing, grooming, and dressing.
- Assist with activities of daily living (ADLs) such as eating, toileting, and mobility.
- Monitor patients' vital signs and report any changes in condition to the RN or Case Manager.
- Prepare light meals and assist with medication reminders (under supervision).
- Offer companionship and emotional support to patients and their families.
- Perform light housekeeping and laundry as needed to maintain a safe environment.
- Document daily care activities accurately in the patient's chart.
Qualifications
- Current Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification required.
- Must possess a valid CPR/BLS certification.
- Strong communication skills and ability to follow written and verbal instructions.
- Ability to lift and transfer patients safely (up to 50 lbs).
- Reliable transportation to and from client locations.
- Pass a background check and drug screen.
- Compassionate demeanor with a focus on patient dignity.
