Responsibilities
- Personal Care Assistance: Assist clients with bathing, dressing, grooming, and toileting as needed.
- Medication Management: Remind clients to take prescribed medications and report any side effects or missed doses to the supervisor.
- Light Housekeeping: Perform light cleaning duties such as laundry, meal preparation, and tidying up the client's living space.
- Companionship: Engage in conversation and social activities to reduce isolation and improve the client's mental well-being.
- Vital Signs Monitoring: Monitor and record pulse, temperature, and blood pressure, reporting significant changes to the RN or Case Manager.
- Transportation: Accompany clients to medical appointments, therapy sessions, or social outings as required.
- Documentation: Maintain accurate and timely daily logs of care provided in accordance with agency standards.
Qualifications
- Valid Certification: Current Home Health Aide (HHA) or Personal Care Assistant (PCA) certification required.
- Education: High School Diploma or GED equivalent.
- Driver's License: Valid driver's license with reliable transportation and a clean driving record.
- Experience: Prior experience in home health or long-term care is preferred but not mandatory for the right candidate.
- Communication Skills: Excellent verbal and written communication skills with a focus on empathy and active listening.
- Physical Stamina: Ability to lift, bend, and stand for extended periods; ability to assist with transfers (hoyer lift experience a plus).
- Background Check: Ability to pass a criminal background check and drug screening.
