Responsibilities
- Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
- Provide light housekeeping and meal preparation services to ensure a safe and clean living environment.
- Monitor and record vital signs, including blood pressure, pulse, and temperature, as directed.
- Administer medications and remind patients of prescribed dosages in accordance with medical orders.
- Provide companionship and emotional support to patients and their families.
- Document patient care activities, symptoms, and progress in electronic health records.
- Communicate any changes in patient condition or concerns to the supervising nurse or case manager immediately.
Qualifications
- High school diploma or GED required; CNA or HHA certification is a plus.
- Valid driverβs license and reliable transportation required for home visits.
- Current Basic Life Support (BLS) and CPR certification.
- Ability to pass a comprehensive background check and drug screen.
- Strong interpersonal and communication skills with a patient-centered approach.
- Physical stamina to lift, bend, and stand for extended periods.
- Must be detail-oriented and capable of following medical protocols accurately.
