Responsibilities
- Assist patients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting.
- Prepare nutritious meals and snacks according to specific dietary requirements.
- Monitor patients' vital signs (blood pressure, temperature, pulse) and report any abnormalities to the case manager.
- Provide companionship and emotional support to patients to improve their mental well-being.
- Light housekeeping and laundry assistance specifically related to patient care.
- Transport patients to and from medical appointments using a personal vehicle.
- Document daily care activities and patient progress accurately in electronic health records.
Qualifications
- High school diploma or GED equivalent is preferred.
- Candidates must possess a valid Driver's License and a clean driving record.
- Must be able to pass a background check, drug screening, and TB test.
- Excellent communication skills and a compassionate, patient demeanor.
- Ability to lift and move patients safely (up to 50 lbs) and stand for extended periods.
- Willingness to complete state-mandated Home Health Aide training courses.
