Responsibilities
- Deliver high-quality patient care via telehealth consultations and remote monitoring systems
- Collaborate with multidisciplinary teams to develop personalized care plans
- Accurately document patient encounters in electronic health records (EHR)
- Provide health education and support to patients and families virtually
- Respond to urgent patient inquiries and escalate critical situations appropriately
- Participate in quality improvement initiatives for remote care delivery
- Maintain compliance with HIPAA and telehealth regulations
Qualifications
- Active California RN license with at least 2 years of clinical experience
- BLS/CPR certification; ACLS preferred
- Proficiency in telehealth platforms and EHR systems (e.g., Epic, Cerner)
- Exceptional communication skills for virtual patient interactions
- Ability to work independently with minimal supervision
- Experience with chronic disease management preferred
- Flexibility to work evenings, weekends, and holidays
- Strong problem-solving skills in remote care environments
