Responsibilities
- Conduct comprehensive virtual patient assessments via telehealth platform
- Develop and execute personalized care plans for diverse patient populations
- Collaborate with multidisciplinary teams for coordinated patient interventions
- Document electronic health records with precision and timeliness
- Provide patient education on chronic disease management and medication adherence
- Monitor and respond to critical health alerts through remote monitoring systems
- Participate in quality improvement initiatives for virtual care delivery
Qualifications
- Active WI RN license with unrestricted practice privilege
- Minimum 2 years clinical nursing experience
- BLS/CPR certification (current)
- Experience with telehealth platforms (e.g., Teladoc, Amwell)
- Proficiency in electronic health records (EPIC preferred)
- Strong critical thinking and assessment skills
- Excellent communication abilities for virtual patient interactions
- Ability to work independently with minimal on-site supervision
