Responsibilities
- Assess patient symptoms via phone calls and determine appropriate care urgency
- Provide accurate medical advice and coordinate with physicians as needed
- Maintain detailed electronic health records (EHR) documentation
- Collaborate with multidisciplinary teams for seamless patient transitions
- Educate patients on self-care and preventive health measures

Qualifications
- Active RN license in Texas (TX)
- Minimum 2 years clinical nursing experience
