Responsibilities
- Conduct comprehensive telephone triage assessments to determine patient urgency and care pathways
- Provide expert clinical guidance to patients via telehealth platforms
- Collaborate with physicians and care teams to coordinate patient transitions
- Maintain accurate electronic health records (EHR) documentation
- Implement evidence-based triage protocols and clinical guidelines
- Educate patients on home care instructions and health resources
- Participate in quality improvement initiatives and process optimization
Qualifications
- Active RN license in Wisconsin (WI)
- Minimum 2 years emergency room or urgent care experience
- BLS/ACLS certification required
- Proficient in EHR systems (e.g., Epic, Cerner)
- Exceptional critical thinking and decision-making abilities
- Strong interpersonal and communication skills
- Ability to work independently in fast-paced telehealth environment
- Experience with triage protocols and assessment tools
