Responsibilities
- Conduct comprehensive telephone and virtual assessments to evaluate patient symptoms and determine the urgency of medical needs.
- Utilize clinical protocols and decision-making algorithms to triage patients to the appropriate level of care, including the Emergency Department.
- Document all patient interactions and assessments accurately and promptly within the Electronic Health Record (EHR) system.
- Collaborate with physicians and advanced practice providers to coordinate referrals and follow-up care.
- Provide clear health education and medication guidance to patients and caregivers based on evidence-based practice.
- Manage high-volume call volumes while maintaining composure and a patient-centric approach in every interaction.
Qualifications
- Current, unencumbered Registered Nurse (RN) license issued by the New York State Department of Education.
- Minimum of 2 years of nursing experience, with specific experience in triage, urgent care, or telehealth preferred.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) required.
- Proficient in EMR/EHR software; experience with Epic or Cerner is a plus.
- Strong verbal and written communication skills with the ability to convey complex medical information clearly.
- Ability to work flexible shifts, including evenings, weekends, and holidays, as part of a rotating schedule.
