Responsibilities
- Conduct thorough patient assessments via phone and telehealth to determine urgency and appropriate care level
- Accurately document patient information using Epic EHR systems while maintaining HIPAA compliance
- Collaborate with physicians and specialists to develop effective care plans for acute and chronic conditions
- Provide clear, empathetic guidance to patients regarding treatment options and next steps
- Monitor patient conditions through continuous triage protocols and escalate critical cases immediately
- Participate in continuous quality improvement initiatives for triage protocols
Qualifications
- Active RN license in Louisiana with minimum 2 years clinical experience
- BLS/ACLS certification required; TNCC preferred
- Proven expertise in emergency room or urgent care triage settings
- Exceptional critical thinking and decision-making abilities under pressure
- Strong written and verbal communication skills with diverse patient populations
- Proficiency in electronic health records (Epic experience strongly preferred)
- Ability to work flexible shifts including evenings, weekends, and holidays
