Responsibilities
- Assess and evaluate patient symptoms via phone or secure messaging to determine the appropriate level of care.
- Provide evidence-based nursing advice and triage recommendations to patients in a calm and professional manner.
- Coordinate patient referrals to specialists, urgent care, or emergency services as needed.
- Document all patient interactions, assessments, and care plans accurately in the electronic health record (EHR).
- Educate patients on self-care management, medication instructions, and preventative health strategies.
- Collaborate closely with physicians and care coordinators to ensure continuity of care.
Qualifications
- Active Registered Nurse (RN) license in the State of Colorado.
- Minimum of 2 years of nursing experience, preferably in a triage, urgent care, or call center setting.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Strong communication skills with the ability to de-escalate sensitive situations effectively.
- Proficiency in electronic health records (EHR) systems (e.g., Epic, Cerner).
- Ability to work flexible hours, including evenings, weekends, and holidays.
