Responsibilities
- Conduct comprehensive telephone and virtual triage assessments for patients presenting with acute medical concerns.
- Evaluate patient symptoms to determine the acuity level and recommend appropriate care pathways (home care, urgent care, or emergency room).
- Document all patient interactions and medical decisions accurately and promptly in the Electronic Health Record (EHR).
- Collaborate with physicians and care coordinators to ensure seamless continuity of care.
- Provide patient education and guidance on symptom management and preventative health measures.
- Manage high-volume call queues efficiently while maintaining a professional and empathetic demeanor.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the State of California.
- Minimum of 2 years of clinical experience in a hospital, urgent care, or telehealth setting.
- Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) required.
- Strong decision-making skills and the ability to work independently in a fast-paced environment.
- Proficient in Electronic Health Records (EHR) systems (e.g., Epic, Cerner, AthenaHealth).
- Excellent verbal and written communication skills.
