Responsibilities
- Assess Patient Needs: Utilize advanced clinical judgment to evaluate patient symptoms via phone and in-person triage to determine urgency and appropriate care pathways.
- Priority Management: Efficiently triage incoming patients to ensure those with critical conditions receive immediate attention while managing non-emergency cases effectively.
- Care Coordination: Facilitate referrals to specialists, urgent care centers, or emergency departments based on assessment findings.
- Documentation: Maintain accurate and up-to-date electronic health records (EHR) reflecting patient interactions and clinical decisions.
- Protocol Adherence: Ensure all clinical practices comply with local, state, and federal healthcare regulations and hospital protocols.
- Patient Education: Provide clear instructions to patients regarding self-care, medication management, and when to seek further medical assistance.
Qualifications
- Licensure: Current, valid Registered Nurse (RN) license issued by the New York State Department of Education.
- Certifications: BLS (Basic Life Support) certification required; ACLS and PALS preferred.
- Experience: Minimum of 2 years of nursing experience, preferably in a clinical or triage setting.
- Communication: Exceptional verbal and written communication skills with the ability to de-escalate high-stress situations.
- Technical Proficiency: Comfortable using electronic health record systems (Epic, Cerner, etc.).
- Availability: Ability to work flexible shifts, including evenings and weekends, as required by the department.
