Responsibilities
- Conduct thorough telephone and virtual patient assessments using evidence-based triage protocols
- Determine appropriate level of care urgency and coordinate with providers accordingly
- Document clinical encounters accurately in EHR systems while maintaining HIPAA compliance
- Provide clear patient education on home care, follow-up procedures, and resource navigation
- Collaborate with interdisciplinary teams to ensure seamless care transitions
- Maintain calm demeanor during high-pressure situations while prioritizing critical cases
- Participate in continuous quality improvement initiatives and training updates
Qualifications
- Active RN license in Texas with minimum 2 years clinical experience
- BLS/ACLS certification and current BLS required
- Previous triage or emergency department background strongly preferred
- Proficiency in EHR systems and telehealth platforms
- Exceptional communication and critical thinking skills
- Ability to work independently with minimal supervision
- Spanish language proficiency highly desirable for diverse patient population
- Flexibility to work evenings, weekends, and holidays as needed
