Responsibilities
- Monitor and record patient vital signs (blood pressure, pulse, temperature) and report significant changes to the Charge Nurse immediately.
- Assist patients with Activities of Daily Living (ADLs) including bathing, dressing, toileting, and mobility support.
- Administer medication as prescribed and document administration accurately in electronic health records (EHR).
- Provide emotional support, companionship, and reassurance to patients and their families during night hours.
- Respond promptly to patient call lights and emergency alarms.
- Maintain a clean, safe, and sanitary environment in accordance with healthcare regulations.
- Collaborate with the interdisciplinary team to develop and update individualized care plans.
Qualifications
- Current Certified Nursing Assistant (CNA) license or Registered Nurse (RN) license in the state of Minnesota.
- CPR and Basic Life Support (BLS) certification required (ACLS preferred for RNs).
- Previous experience in a long-term care, assisted living, or hospital setting is preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and effectively as part of a night shift team.
- Physical stamina to stand, walk, and lift patients as needed.
- Flexibility to work rotating shifts, holidays, and weekends.
