Responsibilities
- Perform comprehensive patient assessments and triage via secure telehealth platforms.
- Develop and implement individualized patient care plans in collaboration with the care team.
- Document clinical encounters accurately and promptly in electronic health records (EHR).
- Provide education and resources to patients to promote health maintenance and disease prevention.
- Coordinate referrals to specialists and community resources as needed.
- Maintain strict adherence to HIPAA regulations and clinical standards.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Maryland.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted with relevant experience.
- Minimum of 2 years of clinical nursing experience (preferably in telehealth or acute care).
- Proficiency with EMR systems (e.g., Epic, Cerner) and video conferencing technology.
- Strong critical thinking, problem-solving, and communication skills.
- Ability to work independently in a remote setting while maintaining patient engagement.
