Responsibilities
- Provide virtual patient assessments via telehealth platform and develop personalized care plans
- Deliver remote patient education for chronic conditions (diabetes, hypertension, etc.)
- Coordinate care transitions between providers and facilities using EHR systems
- Conduct medication reconciliation and virtual medication management consultations
- Monitor patient outcomes through data analysis and adjust interventions accordingly
- Collaborate with multidisciplinary teams to ensure continuity of care
- Maintain HIPAA compliance in all virtual interactions and documentation
Qualifications
- Active RN license in Indiana (or compact state)
- Minimum 2 years clinical nursing experience (telehealth preferred)
- BSN degree or equivalent certification
- Proficiency with telehealth platforms and EHR systems (e.g., Epic, Cerner)
- Strong critical thinking and decision-making abilities
- Excellent communication skills with virtual patient interaction experience
- Ability to work independently while collaborating effectively in a remote team
